Our Services

North Texas Ophthalmic Associates, PLLC provides comprehensive ophthalmological services including:

Appointments

Appointment hours are Monday-Friday 8:00am – 5:00pm. There are things you can do before your visit and bring with you to your visit that will save you time and allow us to provide you better and more accurate prescriptions and treatment.

Paperwork and other information:
  • Insurance:  We will be happy to file with your insurance if it applies.  To do so we must have your insurance information.  Please bring your insurance card(s).
  • Referral authorizations.  If your insurance policy requires you to have a referral from another doctor, it should be obtained and available before your visit.
  • Medical history:  If you have a prepared medical history, it would be helpful.
  • Medicines you are taking.  We need to know both general medications and, in particular, eye medications.  It avoids error if you can bring your actual medications with you.
  • Allergies to medication:  If there are medications you cannot take, it is important for us to know that.
Glasses:
  • Please bring with you all current glasses that you use.  There may be more than one pair used for various purposes.  Even if we give you a new prescription, we want to be able to show the proposed change and help you decide whether purchase of the new prescription is worthwhile.
  • If you have recently purchased glasses from another source that were unsatisfactory, we need to measure and examine the glasses to determine why they were unsatisfactory.  Please bring such glasses with you.
Contact Lens Wearers:
  • Extended wear soft lenses:  Many patients sleep in their contact lenses.  To help us obtain accurate measurements, please do not sleep wearing the contacts for a few (e.g.3) nights before your appointment.  You may wear the contacts during the day, but not at night.  This will help avoid our asking you to return for a second visit for further measurements.
  • Do not wear the contact lenses the day of your visit, if this is at all possible.  Once again our measurements will be more reliable and this may avoid requiring a second visit.
  • Contact lens information:  We need to know what lenses you are wearing.  For disposable soft contact lenses, the best information is obtained from the boxes or packages that the lenses came in.  Please bring these packages if they are available.
  • Contact lens care systems and solutions:  Many contact lens problems relate to the care and sterilizing systems that are used.  Please bring with you the actual bottles of such products you are using.

North Texas Ophthalmic Associates, PLLC accepts Medicare, Aetna, United Health Care, Blue Cross/Blue Shield, SVP and other insurance providers.  Please call us or check with your insurance carrier to determine your benefits and coverage.
